New Patient Form
At SmileLYNN Pediatric Dentistry we believe in minimizing our carbon footprint. In order to do so we utilize electronic forms whenever possible. If you are interested in filling out the new patient forms prior to your child’s visit, we can email you a link. Just be sure to ask when scheduling your appointment! If you forget to fill out the forms beforehand, or prefer to do so in office, please arrive 15 minutes before your scheduled visit in order to complete the electronic forms on our in office tablets.
